X-Rays/Treatment Info from Previous Dentist

Our expert team can provide second opinions on any previous consultations, or help continue a treatment plan you may have paused. If applicable, you can call your previous dentist to have them send over your X-rays to our office before your appointment.

If you are unsure about your complete medical history or do not have access to it, rest assured that our experienced dental professionals will still provide you with the best possible treatment. Our team is trained in the proper assessments and imaging to empower our doctors to give you quality care.

Comprehensive list of medications - examples include heart medications, aspirin, antihistamines, and decongestants. This information is useful to our team because these medicines can affect your saliva, gum health, cause dry mouth or ulcers, and more.
